Request For Transport Form After-School Transport is provided by independent drivers to students attending schools in Markham to the StudentScholars Centre. Independent driver will arrange for child seats which are safe and approved by HTA. 2. Driver will be responsible for picking the child from school, ensuring child is safely brought to the vehicle and fastened with seat belts, help the child to safely leave the car and dropped off at the centre. 3. The vehicle is insured and the driver is a licensed driver *I UnderstandLate Pickup: If the driver is running late, we will inform the parent 1-hour early so that the parent can inform the school for the child to wait in the school office. *I UnderstandUnable To Pickup: If under unavoidable circumstances the driver is not able to pick up your child driver will inform the parent 24-hours in advance so that the parent can make alternate arrangements to drop the child at the centre *I UnderstandSchool Closure/Holidays: Driver will not be responsible to pick up the child on centre & school closure holidays. *I UnderstandInjuries or Accidents Outside the Centre Premises: StudentScholars takes the safety of all of our students very seriously.
