 Transportation & Attendance Policy Acknowledgment

Please read and check each box to confirm your understanding and agreement:

1. Student Absence NotificationI agree to notify StudentScholars at least 24 hours in advance if my child will be absent from school and does not require pickup.*

 2. Late Pick-Up by StudentScholars

I understand that if StudentScholars is running late for pickup, I will be informed at least one hour in advance, and I am responsible for notifying the school to have my child wait in the office.*

3. Inability to Pick Up

I understand that if StudentScholars is unable to pick up my child due to unforeseen circumstances, I will be informed 24 hours in advance and will arrange alternative transportation to the center.*

4. School Closures, Holidays, and PA Days

I acknowledge that StudentScholars does not provide pickup services on school or center closures, holidays, or PA days.*

5. Injuries or Accidents Outside the Centre

I understand that StudentScholars is not responsible or liable for any injuries or accidents involving my child that occur outside the StudentScholars centre.*

6. Independent Transportation Waiver

I release and waive any claims of liability against StudentScholars and its staff related to the use of independent driver transportation services.

I hereby give permission for my child to be transported to and from the StudentScholars program by an independent driver. I have read and understood this waiver and agree to the terms stated above.*
